2022-2023年入学手续
THE FOLLOWING ARE THE STEPS TO FOLLOW FOR NEW STUDENTS
对于新生
STEP 1: Prepare the following documents (SOFT COPIES for ONLINE Enrollment or HARD COPIES for ONSITE Enrollment): 准备些文件(对于线上录取,准备电子文件;对于现场录取,准备打印件)
- Original Birth Certificate from Philippine Statistics Authority (PSA) 学生Philippine Statistics Authority(PSA)出生证的原件
- Original Report Card (Form 138/School Form 9) from the Originating School 学生原学校成绩单的原件
- Original Certificate of Good Moral Character from the Originating School 学生原始学校的良好品德证书原件
- One (1) Valid ID of either parent that indicates the address of the present residence. 父母有效的身份证表明着家里现在固定的地址
- ID pictures (passport size, white background) 证件照(护照尺寸、白色背景)
- Baby Booklet/Medical Record 小本本/体检记录
STEP 2: Schedule an Interview Appointment with the school's Guidance Counselor. 跟辅导员预定个面试
- a) For ONLINE Enrollment: Choose a date/time by filling out this TSA Online Interview Appointment form to schedule an online appointment 线上:先填好上面的链接才选个日期跟辅导员预定个面试。
- b) For ONSITE Enrollment: Choose a date/time. Pay a visit to our school, Tiong Se Academy, Inc., then proceed to the Guidance Counselor's Office. Minimum health protocol must be observed. 现场:选个日期和时间来中西学院,然后直接去辅导员的办公室。必须遵守最低限度的健康协议。
STEP 3: During the Interview with the Guidance Counselor, the parent or guardian has to be with the child at all times. The interview will be STRICTLY CONFIDENTIAL. 跟辅导员的面试当中,父母或监护人每时每刻陪着孩子。面试将严格保密。
STEP 4: After passing the interview and screening process, submit the files or documents mentioned in Step #1 to: 通过面试后,可以提交了第一步所需要的文件。
- a) For ONLINE Enrollment: Send the soft copies of the documents to admissions@tiongseacademy.edu.ph. 线上:所有电子文件提交到admissions@tiongseacademy.edu.ph.
- b) For ONSITE Enrollment: Submit the hard copies of the documents to the school's Records Officer. Fill out and sign the Registration Form and Enrollment Policy Form. 现场:所有打印件提交给记录员。填好报名表和入学政策表。
STEP 5: Accomplish the Learner Enrollment Survey Form (LESF) as prescribed by the Department of Education (DepEd). 完成教育部规定的入学调查表。
STEP 6: Inside the LESF file, choose the preferred payment setting and schedule. To know the matriculation fees, please: 入学调查表里,优选交费日期和设置。如何知道学费请:
- a) For ONLINE Enrollment: Send an email to accounting@tiongseacademy.edu.ph. Take note that the school operates in limited hours. We will try to respond as soon as we can. 线上:发信息到 。请注意学校只操作一段时间。学校会尽量回复你们的信息。
- b) For ONSITE Enrollment: Ask for the details from our school's Accounting Officer. 现场:直接跟会计主管问。
STEP 7: Follow the instructions for the desired payment method: 按照所需付款方式的说明进行操作:
- a) For ONLINE FUND TRANSFER (第一付款方式:在线资金转账), click here to see the details.
- b) For PBCOM OVER-THE-COUNTER (第二付款方式:PBCOM 柜台付款), click here to see the details.
- c) For ONSITE PAYMENT AT TIONG SE ACADEMY, INC. (第三付款方式:现场付款(中西学院)), click here to see the details.
STEP 8: Upon payment verification, the school shall issue an official receipt via: 付款核实后,学校映出具正式收据:
- a) For ONLINE Enrollment: An acknowledgement email. 线上:确认电子邮件
- b) For ONSITE Enrollment: A hard copy of the receipt. 现场:收据硬拷贝
STEP 9: The Student Affairs shall assign the student his/her TSA Google Workspace Account for HyFlex Learning use. 学生事务部应为学生分配他的TSA Google Workspace 的帐户HyFlex Learning使用的。
STEP 10: Sign in to the student's TSA Google Workspace Account by following the procedure in this video tutorial. 按照这段视频(Video Tutorial)签到学生的TSA Google Workspace帐户。
THE FOLLOWING ARE THE STEPS TO FOLLOW FOR PREVIOUS/OLD STUDENTS
对于老生
STEP 1: For ONSITE Enrollment, fill out and sign the Registration Form and Enrollment Policy Form. 现场录取,填好报名表和入学政策表。
STEP 2: Accomplish the Learner Enrollment Survey Form (LESF) as prescribed by the Department of Education (DepEd). 完成教育部规定的入学调查表。
